Before proceeding with the classification of gynecomastia by Simon, we will refer to some special cases

Unilateral gynecomastia (on one side)

Increasing in size puffy nipples

Gynecomastia in a bodybuilder.

Classification of Gynecomastia by Simon (1972 Salt Lake city AAPS)

According to Simon et al the treatment and the type of surgical technique depends on the gynecomastia class, meaning the type of gynecomastia.

Class I: small visible breast enlargement, no skin excess

Class IIA: moderate breast enlargement, no skin excess

Class IIB: moderate breast enlargement with extra skin (skin drop of Class I)

Class III: Marked breast enlargement with significant extra skin (skin drop of class II, III)
